Baptism

Baptisms are scheduled for the first and third Sunday of each month, and are scheduled for 12:30 at Saint Mary and 1:00pm at Holy Spirit.  To begin the process, please fill out the information below and we will contact you.  First time parents and godparents may need to attend one Baptism preparation class.  Godparents will need to fill out the Sponsor Affirmation form, available below..

    Baptism Request

    If you are married, your maiden name is the surname you used from birth, prior to its being legally changed at marriage.  We ask for this information because the mother's maiden name is required to be on the child's baptismal certificate and recorded in the official parish record of the baptism.
