Baptism


Sponsor Affirmation Form

To begin the process, please fill out the information below. When the form is received, a member of the Parish staff will contact you to schedule a meeting between the parents and one of the priests or deacons. This meeting will cover the importance of the sacrament and guidance on bringing up children in the Catholic faith. This meeting must be held prior to scheduling the date of the baptism and selection of the godparents. Please choose one church when selecting your preferred dates.

Selection of Godparents
Please do not invite anyone to be a godparent until after the meeting with the priest or deacon. In considering potential godparents, please note that the Church requires those seeking to be admitted as a godparent/sponsor "lives a life of faith which befits the role to be undertaken." This phraseology means that the candidate for godparent/sponsor; A) sincerely believes and strives to put into practice the Word of God as taught by the Catholic Church; B) regularly takes part in the Mass on Sundays and Holy Days of Obligation; and C) if married, was married in accordance with the requirements of the Catholic Church.

    Baptism Request

    If you are married, your maiden name is the surname you used from birth, prior to its being legally changed at marriage.  We ask for this information because the mother's maiden name is required to be on the child's baptismal certificate and recorded in the official parish record of the baptism.

    Once you have your discussion and meeting with the Deacon, a date and location for your ceremony can be scheduled at your convenience.
